City License Applications / Permits / Forms

Please fill out the following forms that apply to your situation and return them to City Hall at 118 Center Street West:
  • Application for Conditional Use (PDF): This is the application you file for conditional use; the cost is $175.
  • Building Permit: Minimum fee is $150; fee increases depending on building.
  • Board of Zoning and Appeals Variance or Appeal Application (PDF): This is the application you file to apply for a variance or appeal to zoning regulations, the cost is $250.
  • Certified Survey Map Approval Application (PDF): Application to have a certified survey map approved by the Amery Plan Commission; the cost is $100 plus $10 per lot created.
  • Driveway Permit Application (PDF): Cost is $30.
  • Dumpster Permit Application (PDF): This is the form used to get a permit to place a dumpster in the public right-of-way for temporary use. For example: You need a dumpster to re-roof your home and place the dumpster in the City Street. Cost is $25 for a two week permit.
  • Fence Permit Application (PDF):  
  • Merchant / Transient Application (PDF): Cost is $20.
  • Rezoning Petition Application (PDF): Cost is $200.
  • Sign Permit (PDF):  This is the application you file to apply for a sign permit.  The cost is $100 less than $5,000 improvements; $175 greater than $5,000 improvements.
  • Signal Receiving Antennas (PDF):  This is the application you file to apply for a signal receiving antenna.  The cost is $50 less than $5,000 improvements; $100 greater than $5,000 improvements.
  • Site Plan Approval Application (PDF)
  • Swimming Pool Permit (PDF):  This is the application you file to apply for a swimming pool permit.  Cost is $200. 
  • Zoning-Permit-Application:  This is the application you file to apply for a zoning permit, the cost is $75.
